    thrombotic thrombocytopenic purpura (ttp) pregnant patient with covid-19: a case report

    introduction: thrombocytopenic purpura (ttp) in pregnancy is characterized by high blood pressure, creatinine, and bun levels. the risk of ttp in pregnant women with covid-19 is considered a potential threat to maternal and fetal health. patient description: a 40-year-old g3p2l2 woman with bp=140/90 mm hg and covid-19 was referred with symptoms such as chills, nausea, headache, shortness of breath, and body aches.

    ttp was diagnosed according to the blood pressure =140/90 mm hg, cr = 1/6, bun = 47, ldh = 1995, and abnormal lft, and emergency cesarean section was performed to prevent fetal injury. in the recovery room, the patient’s level of consciousness was 12, so the patient was transferred to the covid-19 icu, where she had symptoms of tachypnea and tachycardia, and the conjunctiva was icteric.

    plasma milling was started with 30 cc/kg/day and continued for 5 days. the patient was monitored in the covid-19 icu for 10 days.the patient was discharged from the hospital after his vital signs, laboratory indices, and consciousness levels were normal. conclusion: covid-19 and ttp overlap in some clinical features. therefore, the differential diagnosis may be challenging in covid-19 pregnant women presenting with hypertension, proteinuria, thrombocytopenia, or elevated liver enzymes.
